I always advise anyone due to claim DLA to keep a "diary of a week".
From the time you get up until you go to bed, write down what you have done, how easy/hard it was to do, and if you couldn't manage to do something then note that.
If it caused pain etc., then write it down - then you'll remember when you come to do the form and it will help any adviser.
If you have memory/function problems, then get it noted.
It's amazing how many things people miss when filling out a DLA form - give them loads of info, attach sheets of paper if you need to - but most important - get someone professional/very experienced in benefits to fill it out with you.
The DWP are tightening the criteria - especially with "mental health" - and more and more are being rejected.
That way lies a much better chance of success, first time.
If you fail, appeal straight away - forget "reconsideration", as it's a waste of time. All appeals go for re-con anyway.
