Quick direct debit mandate query

Stupid questions but I just want to double check
Usually you can set a direct debit up over the phone
Anyway, it says "reference number" - is that just what I want to show up in my bank statement when they take the money?
Then it says "address" and starts with "To: The Manager" so I put "To: the manager Halifax bank/building society"
then the address for my sort code from here (Branch name, then PO address)

Your post doesn't read well (probably not your fault), but what I think you're saying is the form is asking for a 'reference' as well as sort code and account number?
If that's the case then, providing your account is with one of the main banks* just leave it blank.
* Some building societies running psuedo current accounts (eg the YBS 'Cash Transactor' account) use a common (collection) account number and a unique customer reference number.0 -

address should automatically be added by bank as sort code provided can get it off there
put your 'holding branch' address in
Reference for your use so you can leave it blank0 -

I don't think that is what the OP is chatting about from looking at the sample direct debit forms below

It would appear that the "reference number" is for office use only rather than being a Building Society customer number used with a collection account
