Application question

Hi
I have received my application through the post where I need to read the contract and sign t&c's etc and post back along with my ID.
However, amongst the paperwork there is a form titled 'For Social Use Only' and it has loads of boxes that have already been ticked and at the bottom it says 'signed on behalf of Nationwide' and then a box for the branch stamp.
I tried to call them to see if I need to return or keep but I was on the phone for over 7 minutes with no answer and just on hold - I cant hold for that long on an 0845 number on my mobile!
Would anyone have any idea what I need to do with this form?
Thanks

If you're not sure, why not take a photocopy to keep for yourself and return the original? You've covered all bases then.0

Lol I was thinking that so I scanned a copy and will keep it on my PC just to make sure.
