Thanks for all the tips and taking the time to read the letter - I will take on board your comments and cut it back to the minimum. I was tempted not to send it at all and just sit tight but felt it better to let them know what I intend to do so they don't start advertising the place etc.
I remember from last year that they called me after a while and scared me a bit into thinking I was going to get kicked out so I paid up - will try not to cave in this time!
I've not as far as I'm aware been issued with a S21.0 -
Dear Mr landlord,
Thank you for the offer to renew my contract when the fixed term expires on xx/xx/xx. I feel however that it is simpler for both you and myself to let the contract move to a monthly periodic tenency.
While writing, may I take this opportunity to remind you of the outstanding need to replace the boiler. When last repaired on xx/xx/xx the engineer removed controls such that the heating/hot water are no longer controllable. I understood this to be a temporary state since the boiler was due for replacement.

Send this to the landlord, and a copy to the agent.
In matters like this, the less you say the better. Just state what you want - no explanation, no reason, no excuse. And supported with history (if you've had multiple conversations/letters/emails/repairs on the boiler, list them all with dates. etc
Don't mention the rent rise at all. If nothing more is said of it, your periodic tenancy will be at the old rent. Put the ball back in the LL (agent's) court, but without actually requesting they respond.0
