Bus fare's

When I was on working tax credit I had a NHS card which I could claim my bus fare's for hospital appt's. I've lost this card and am no longer claiming WTC just ESA. Can I get a similar card if so where do o apply for it?

If you are on ESA (income based) you can get bus fares for Hospital appointments by showing your award letter.
If you are on ESA (contributions based) what you can try is to fill out an HC1 from (available from Drs, Dentists, Opticians and pharmacies) to apply with help for health costs on a low income basis. You will either get an HC2 (full help) or an HC3 (partial help, which means you will have to pay so much before you can get help, including travel fares).0 -
