Severing Joint Tenancy

Evening all,
Does anyone know where I can get a draft 'notice of severance' letter and a draft 'without prejudice' letter. I want to severe a joint tenancy and apply through the Land Registry using form 'SEV' to become tenants in common.
If anyone can help or has any experience of this I would really appreciate hearing from you.

If all the parties agree to the severance you just use the LR SEV form.RICHARD WEBSTER
