Doctors charges

I have been told by my GP that he requires a payment of £89-00 per insurance form I need signing for my bank for claiming for a credit card and loan, 2 forms, whislt I am recovering from major surgery. I have been on the BMA website, thou not updated since October 2009 it states,
Certificates without examination
a) Straightforward certificate of fact - £15-50
b) More complex certificates - £26-00 to £55-00
Has anybody had to have an insurance certificate signed lately and how much did it cost ? These are not unfit for work notes.
Also a third company sent me a claim form which all I had to do was sign and return, then they would do the rest all costs taken care of.
How can a GP justify charging so much, one of the claims is only 1 page and how can one insurance company be so different to another ?

Your GP can charge what he wants - the BMA can only suggest charges, not enforce them.
They can make these charges as they are doing this (completing isurance forms etc) outside of the NHS - you are paying "private" rates.0 -
£89 is the normal charge for a Doctors Report - not a signature on a form. he may be getting them mixed up?
But he is free to charge what he likes.
However, depending on the insurance - the company concerned may pay the fee. Cheers guys, it still seems very steep to fill in a few dates and a medical diagnosis, 10 mins per form max, £534 p/h whilst they are already on a salary !0

Some doctors charge a person beside no insurance or on medicare a less rate than they bill the insurance company.0
