PPI Totals lets see who are to blame

Lets put together a list off sucessful PPI claims which i will put together with everyone one on this forums help.
What i would like is.
1. Name of Bank/Lender
2. Date of complaint.
3. Date of redress (when you got letter with an offer)
4. Amount of the loan
5. Amount you recieved back
I hope of you can give me this information i can put it together and help others. Not only this but it will show everyone who are the worst companies and how long it is taking to sort out. I am in the process of getting back my PPI and know this would off gave me a lot of encouragement and hope of getting my money back.
Please just put the 5 pieces of information i have asked for so that it keeps the thread as small as possible and it is quick for users to scan down.
