Double glazing steam and condensation

I was hoping someone could point me in the right direction. I have a small box room with double glazing but is horrific, moisture, dirt, condensation etc. The rubber seal doesn't even look like it fitted correctly when first fitted (I have only recently moved into the house).
Can the glass just be replaced or do you have to buy whole units. I live in NI and am still having trouble finding companies who could fix this. The only ones that appear are your average double glazing installer which don't do repairs.

You can just buy the double glazed glass on it's own, rather than the whole unit with frame and all.
Any glass shop should be able to get you one, although I know nothing about NI.
After googling Northern Ireland glass, there are quite a few,
here is just one of them

A local glazier should be able to fit a new DG unit for you, the units are not expensive. Ask one to come out to take a look and give you a price.0
