Dla form re-assessment

I have asked for my husband to be re-assessed on dla.

The section which asks how his disabilities effect him - I have typed out a detailed report on the effect his disabilities have.

Do I have to write this in the box OR can I send in the typed version?

Would it go against him being typed?

The rest of the form is hand written by myself as his carer.

    You can put something like 'see additional information attached' in the box for question and make sure that the page you've typed up is clearly marked as answering that section.

    I did this with a lot of the answers on my form as I wanted to provide more information than I could fit in to the answer box.
