returning to work after long term sick

Hi i have been on the sick for just over 12 months due to personal health problems,i now feel i am ready to return to work,this is the first time i have ever been unemployed in 21 years, i'm not sure what to put on the application forms and how this will effect me? i don't want the next employer thinking i have a bad sickness record. also, am i entitled to any benefits once i sign off employment & support allowance?

Re: Benefits - It is very difficult to give you an idea about benefits without more information about your circumstances. You could look at turn2us to do an online check. :-)
In terms of returning to work - I would concentrate on the positives, the challenges you have overcome and beaten, your enthusiasm for getting back into work and the skills you have picked up over the past year, even if you have been sick you may have been battling for benefits, dealing with a reduced household budget or looking after children etc... all of this should be of interest to any employer!
