£40 is done through the proper channels. You don't need an accountant to set up a Ltd Company. You can buy one off the shelf or create one in a name of your choosing and all you need to provide is the name of a director, company secretary and address for the registered office which could be your home.
If I were you, I'd be changing accountants as they've clearly ripped you off.0
