yep have seen it frequently, particularly around the busier times - eg christmas. transactions 'time out' and the retailer processes it again. the first transaction didnt actually time out though - result - double debit pending
double debits from a retailer sitting pending for 3 days. customer has no money to access because it wont drop off. very frustating for them naturally.
