IB form 50

Hi to everybody
After a long wait i was awarded IS £57.45 per week and have
finally recieved confirmation today that i am entitled to IB £70.05 per week after waiting since early September. I assume that i will either loose the IS now or it will be lowered beacuse of IB.Also today i have recieved form IB 50 to fill in.Does anybody have any guidance as to filling this form in they haven't asked me to supply MED4.
can anyone give me some advise??

You should fill this in as a view from your worst day. In an ideal situation it would be best to get some help from your local Citizens Advice Bureau or Welfare Rights Service who should be able to help you fill it in and advise you etc. I have just had mine through and got someone to help me - couldnt have done it without them - as they have filled them in quite a few times they knew what to put where etc.
Depending on the illness that you have is there any local support groups that may be able to offer advise - I say this as I have CFS/ME and have had help from the local group of thatWeight Loss - 102lb0 -
What is the MED4 form?- I have an IB50 to fill in and they have asked for this form. I rang the GP's and they said I have to come in and see the doctor to get it.Jane

I think its a more detailed sick note which covers you for 6 weeks. You get it from the doctor and its free.0

Does this mean that I will only get IB for 6 more weeks then? OR do I go back to 13 week notes again. Yeah your GP will know which form you are on about and it covers you for a longer time than the others. I had to send a MED4 form in with my IB50 last year and I dont have to this yearWeight Loss - 102lb0

My hubby had one which im almost certain covered him for 6 weeks, he then sent that off and got a reply saying he does not have to send sick notes in for another year.
Before he sent the form in his doctor was giving him 8 week sick notes
But anyways dont worry about how long they are for, they are just a more detailed sick note from what I remember. Someone with more knowledge might be able to clarify but arent they just another thing which everyone who claims has to fill in ?0 -

I don't know what the MED 4 actually says, but I think you have it as well as (not instead of) an ordinary sick note for the sole purpose of sending to the DWP, you don't have to send it to your employer. My husband sent his in with his first IB50 and then was told he didn't need to send in any more sick notes. He has since sent in a second IB50 (he didn't need another MED4) and has continued to receive IB.
A couple of tips about the IB50, as someone says, fill it in as though it is your worst day, for example if you can only climb stairs ocasionally then you can't climb stairs.
It is a good idea to get a welfare rights officer at the CAB to help you fill it in.
Also, my husband sent in with his second IB50 things like copies of prescriptions, hospital appointments etc. Don'yt know whether this helped or not but it certainly can't do any harm.
