ESA basic query

Recently started recieving ESA.
They put me on Contributions Based ESA.
Was this their choice? or mine? I can not remember them asking me.
And i dont understand the diff with that and Income Related ESA.
Medicine on NHS is free only for Income Related ESA?
And Community Care Grants are for Income Related ESA.
Could i switch to Income Related ESA?
My medical bills will be quite substantial during the year i think.
