I remember getting a letter from a CC once telling me that I had underpaid and needed to make a HUGE payment like ... yesterday. Which was odd as I had cleared the balance at the beginning of the month as normal
, when I went into the branch with the letter and saw the Bank Manager he looked really sheepish!

It's because most of these things "grow" rather than are put through a proper analysis. I know a Poll Tax system (it's a while back) that had no marker to say the person was dead, so people used to put (DECEASED) after the name when they were informed.
Great when it's a person reading it, not so good when the automated bills went out...
