I remember getting a letter from a CC once telling me that I had underpaid and needed to make a HUGE payment like ... yesterday. Which was odd as I had cleared the balance at the beginning of the month as normal
, when I went into the branch with the letter and saw the Bank Manager he looked really sheepish! Do not meddle in the affairs of Dragons, for thou art crunchy and good with catsup

It's because most of these things "grow" rather than are put through a proper analysis. I know a Poll Tax system (it's a while back) that had no marker to say the person was dead, so people used to put (DECEASED) after the name when they were informed.
Great when it's a person reading it, not so good when the automated bills went out...
