EHIC help please

Hi
I am applying for a passport for or grandson who we have residency for via the courts. I have filled in the passport side of things but I am unsure of the EHIC.
It states that the appliction must be filled in by persons over 16, so does this mean that I fill it in for myself and then add my grandson, or should it be for my grandson only, its needed for him to go on a school trip to Paris in June.
I have phoned the number on the front of the form but this is only an automated service for applying for it by phone, there is no-one to ask for info.

From memory it can only be filled out by over 16's as the card is issued against a National Insurance number (and obviously they don't have one)
For under 16's it is filled out with the Parent/Guardian's National Insurance number, I don't remember if it needs to be the Parent/person with parental responsibility whose number is needed though.0 -

Applicants under 16
To obtain an EHIC for a child under 16, the parent or legal guardian should apply as the main applicant and include the child as a dependent. "0
