Need help with applying for books and equipment through learning support

Hi there,
I just wondered if anyone could answer a question I have had before I go back to university.
I have been accepted for DSA and help with equipment and books. Though I have been given much support, for some reason I have never applied for books or help with other items. Apparently for this I need to fill in some forms , and then get the money given back after I paid for them. I dont think, if not cannot find these forms. Could anyone advise where I could get them from , or give any other assistance ? Many thanks

Got to this page http://www.direct.gov.uk/en/DisabledPeople/EducationAndTraining/HigherEducation/DG_10034900 and about half way down there is a link Download claim form for reimbursement of costs through Disabled Students' Allowances 2010/11 (PDF, 68K).
Fill this form in and send it to Student Finance. I always kee a copy for my records incase they go missing.

i've sent my internet access ones in. I'm just waiting for them to send me the £60....bet it takes months! I think I'm going to batch buy my printer ink and paper in and send my receipts off all at onceMoney money money.
