disablity working tax credit: how long is it paid for?

Hi all
not posted for ages as have been busy working which has been really good. I work 21 hours a week and my wages are currently topped up by the return to work credit and WTC made up of the over 50's element and a disablity element. I know that the RTW and the over 50 payments are only paid for 12 months but what about the disablity bit? Does anyone know? I need to make some desisions about my working hours very soon and for health reasons would like to still do 21 hours but if I am to lose all my benefits I need to have a good look at my budget to find out how i will cope on reduced income. thank you.

thanks, does this mean I have to have a medical? I was on ESA before getting the job and this meant that all I had to do was write this on my claim. I am unsure what I will have to do when I renew it. I am still on medication and will have to be for the rest of my life.0
