Medical cover in Europe, having already left the UK

Hi all, after some help please!
have been trawling the internet to no avail to sort this problem out. We are in Italy on an extended trip, we took out insurance before leaving, an annual policy with single trip cover of 100 days. The short version is that are plans have changed and we have just remebered that our 100 days are nearly up. Our insurance company wont extend cover and we are struggling to find any cover at a sensible price that will cover us without returning to the UK to restart our trip!
World nomads will quote around £270 for 90 days (its cheaper to fly home for a day and come back!)
We dont need the 'travel' side of the policy and we are covered by what the EHIC covers us for, we basically need cover against repatriation or some kind of medical problem that goes beyond the EHIC. Anyone have any thoughts!!! Our cover runs out on Xmas Eve and we would have a better Xmas knowing we are covered!!
