Able-Labels.
Although I don't know where my mum gets the ones she used to put in the jumpers she knitted the boys: "lovingly knitted by Grandma" and they had a logo of knitting needles on. Can't see anything like that on the Able-Labels website but it might be worth asking them if you wanted something like that added.
