Bookmarks and header bar missing. (Firefox)

The bar across the top has disappeared? It's the one which shows my bookmarks and various other options. How do I get it back?

    Tried right clicking on the blank toolbar, select customise and then restore defaults?

    The menu bar was un-ticked because the small icon tick box wasn't ticked.
