Halifax - Error on Credit Report

Hi
I recently opened a new credit card account with the Halifax and correctly filled in the forms to set up a direct debit to settle the full amount each month from my bank account.
Last month, I received a nasty phone call from them saying I was in arrears, followed up by at least 2 nasty letters.
After a number of phone calls, it was apparant that my bank had refused the payment because Halifax failed to ask them to set up the direct debit in the first place.
I had no statements telling me what I owed nor any indication that the direct debit had not been set up.
Halifax charged me a late payment fee (£12) plus a small amount of interest. I settled the entire balance by debit card as soon as I got the call.
After compaining vigorously to a number of people by phone, I managed to get Halifax to refund the late payment charge, which they've since done.
The problem is that today I was notified that a 'derogatory' entry for late payment has been put on my Equifax credit file by the Halifax. As I've done nothhing wrong and resolved the situaiton as soon as I was made aware of it, I object to this and want to get this entry removed.
Can anyone advise how I might be able to get this done and whether there are any other agencies I might need to check in addition on Equifax?

Ring Halifax and ask them to adjust the credit file accordingly.
If they consider themselves to be at fault they will.
Did they confirm the direct debit was in place (letter or message on statement)?0 -

The problem was they didn't send a statement until after the payment was declined. Don't know why. I requested a duplicate which they then sent.
Had I seen a statement which didn't mention a DD, then I'd have called up and paid by debit card!
You mentioned 3 agencies - I'm assuming that Equifax is one but do you know the names of the other two?
