Were you satisfied or dissatisfied with their response?
If you were unhappy with the response, you have been invited by The Parliamentary Health Committee to submit written evidence for its inquiry into Complaints and Litigation.
The deadline for submitting written evidence is noon on Tuesday 21 December 2010.
For guidance on the submission, please view the Parliament Health Committee Complaints Inquiry website.
The committee prefers emailed submissions, please view the website for the email address.
For those without access to a computer they should send a hard copy to:
