Help with HC1 Form?

Hi, i'm trying to fill in my HC1 Form. I already have an NHS Medical exemption card for free prescriptions (which i *think* i filled a HC1 form in for a few years back) but this doesn't entitle me to help with dental or optical help. I'm just a bit confused, on the form you don't actually tick what you want help with ie. prescriptions, dental costs, wigs, eye tests, glasses etc so how do they know which one you're applying for?

Also it asks about savings, all together i have around £2,000. Should i declare this on the form or could it affect my benefits? I recieve IB and DLA high care, high mobility.

    You need to declare everything on the form, income from work, benefits, student loans, any saving at all and they work it from there.

    The form you filled in before was one for your doctor to get the medical exemption card, HC1 forms are universal across the NHS, if you get an HC2 certificate back you will get help with prescriptions, wigs, dental, optical and travel costs; you don't need to choose what help you need.
