How do I ask for a fit note?

I know how ridiculous this sounds but I don't know what to say and it's really making me panic.

I want to spend as little time in the doctors as possible. I'm suffering from anxiety and depression (or at least that's what I think it is) and I am dreading sitting one on one with the doctor :(

I don't know what to say, where to start or how exactly to ask for a fit note or if they will just offer me one or what.

I have been diagnosed with post traumatic stress disorder, depression and borderline personality disorder a few years ago. I have since moved hundreds of miles away and only just registered with a doctor here after two years. I presume they will have to send off to get my file from my old doctors/hospital.

I'm also really worried that I won't get a fit note even though I'm sure I will as things are really bad at the moment. I can't wait until I've been and I can stop panicking about the whole thing.

Any help would be really appreciated.

    Could you write it down and ask them to read it on your appt?

    As soon as you registered with the GP they will request your notes from your previous GP. Try and not work yourself into a state about going there. Drs have proberly seen it all before, just be honest and try and explain how you are feeling. I have spent 10 mins with a Dr and just cried because i couldnt explain how i was feeling, they will see how you are scared/upset etc and ask you questions - just try and answer them the best you can. Take a friend in with you if it will make you feel better. Good luck and and try not to worry. Drs are there to help you get better.
