BT error triggered bank charges

I set up a monthly agreement with BT to pay my bill at £18.

I few weeks after the agreement, BT debited over £40 from my account. This was returned and triggered a bank charge.

I called BT and they said they cannot refund the charge unless i write to them and provide my bank statement.

Does anyone know the address to send this to?

    Changing to Monthly Whole Bill Direct Debit would prevent such occurrences in the future.
    Time has moved on (much quicker than it used to - or so it seems at my age) and my previous advice on residential telephony has been or is now gradually being overtaken by changes in the retail market. Hence, I have now deleted links to my previous 'pearls of wisdom'. I sincerely hope they helped save some of you money.
