Pensions and Tax Credits

Hi,

Can anyone help on this question:-

How much to i declare on my tax credit form below is my current situation:-

I have a basic salary of £23400 and from this my employer deducts 10% for my pension. (£21060 left after the pension deduction) i also pay for private medical care for the family £40/month from my monthly salary.

My partner works part time and earns £1000/year, we have three children and receive child benefit for them and we do not have any child care costs.

I would appreciate some advice on this, as i cannot seem to get a definitive answer from the revenue.
