Employment Support Allowance Questions

From daughter:

I've recently been in hospital for an operation, and had to stay in overnight. When I was going for the consultations I was under 18 so was covered by my dads benefits and we could claim transportation costs by showing his NHS Tax Credit Exemption Certificate.

I've recently turned 18 so am no longer covered by my dad's NHS Tax Credit Exemption Certificate for claiming transportation costs when visiting a hospital. The other day I had an operation and had to stay in overnight.
I'm on Employment Support Allowance due to the problem I was having the operation for, and was wondering if I can claim back the money, and how I would do so. I have a form to fill in from the hospital.



ALSO
I have £25 from a mystery shop I did a few weeks before the operation, and am unsure of how to declare it. Do I actually have to go into my local job centre plus, or can I do it by phone? I'm currently on crutches and it is a lot of effort to move around as I'm not allowed to put more than touch weight on my right leg so it would be difficult to get to the centre.

    She may be entitled to claim transport costs via the NHS Low Income scheme. She can find the information on eligibility, how to claim and how to get refunds HERE

    Is daughter on income based ESA? ESA (IR) automatically entitles you to help with travel costs.

    I'm on the benefit ESA, it's my only income, sooo is that a yes it's automatic? In which case how do I show them that I'm on it?

    It should say on the award letter whether it's income based or contributions based. Income based gives you automatic entitlement; contribution based doesn't.
