HMRC Tax Credits and Form M1

In 2003 when i rang the Tax Credits dept to query my award ( I felt I was being overpaid) I was told that a 'Form M1' would be raised to investigate the matter.
can anyone tell me if such a form exists or did exist in 2003 to instigate an investigation??
