Hi and welcome
The reclaiming letter template have been replaced with a questionnaire on this site, but if you want to write a letter its best to in your own words by just doing this:
Request for all your PPI, plus interest paid into the account.
Give as much info as possible on why your making the reclaim.
Ask that they comply by 8 weeks.
Then confirm if you do not receive a favourable response, you will consider taking further action in order to retrieve your refund, such as with the Financial ombudsman service (FOS) or the courts.
But if you want to complete the questionnaire you will find this here on stage one of reclaiming:
http://www.moneysavingexpert.com/reclaim/ppi-loan-insurance#reclaim
Complete and send to whoever responsible for setting up the policy.
Keep a copy for yourself as well, in case you need to refer to it at some point.
As usual they have 8 weeks, but may ask for more time.
If you want to do one of the other or both of the above, that is up to you.
Good luck and please keep us posted, cheers.;)The one and only "Dizzy Di"0
