sorry to ask a stupid question but hope you can help. i need to fill in a direct debit form from my dentist to set up dental insurance, i have not filled in a dd form before. on the form it asks for FEE AMOUNT do i put the totally yearly amount or the monthly amount which i will be paying. Many thanks.

Depends if you are paying monthly or yearly. Suggest you ask your dentist who I guess gave you the form?0
