Filling out form HC1 ?

I am going to try and claim back my travel expences to London where I had to go twice, once as an outpatient and then as a day case inpatient ( not actully me but my 1 yr old son)

On the HC1 form it asks for patients info, but my son is only 1 and its free for him to travel, and there is no where to put my details.

Can I still claim as he could not have gone on his own!

Getting really confused now. :o

Ive also got form HC5 is this the one i claim on??????

    hi,
    Information on who can claim and what you can claim for with regards to travel costs can be found HERE on the NHS website.
    You need to fill out a HC5 form which is available from the link above under 'how do I claim a refund'.
