Need help with Disablement element of working tax credits

My friend has got a letter asking for information from HM Revenue
Please provide original documents showing start and end dates of any benefits you recieved prior to claiming the Disablement Element of working tax credit.
Do you suffer from a condition that puts you at a disadvantage in getting a job? If so please specify what that condition is and how this impacts on your everyday activities.
Provide original documents not photocopies.
He has been working and claiming working tax credits/ with a disablement element for nearly four years. at first they asked him if he was suffering from any disabilites that put him at a disadvantage from getting certain jobs. He put yes:-
He has rods in both knee joints and has several lung colapses which led him to having half removed and them stitched to the chest wall to prevent further colapses.
He called to confirm was he entitled to Disablement Element of working tax credit, they said yes as they looked at his NHS records, and he got a NHS card also in the post.
He has called them about this letter he got and i can not understand it at all, they said for him to contact Citzen advice (he has tried calling but can not get tho to them for advice) he was told he has to get a DLA form and take it to the doctors (and they cancelled his tax credits there and then as he has not claimed any benefits for 20 years which was JSA, she asked how he manages and he said well i work" which they should know as he gets working tax credits.:(
Has any one got any advice in what he should do? And he is also worried that he may have to pay some back" because of this, but the lady on the phone said she can see his entitled to the Disablement Element of working tax credit.
I just do not understand it at all and he is dyslexic and also can not understand it so i try to help where i can, but i cant on this as i do not know anything about it.

The HMRC cannot access your NHS medical record. What do you mean by an NHS card?
Have a look at the Disability Helpsheet found on the HMRC website HERE to check that he meets all of the conditions set out.
By the sound of the letter they just want to make sure he still qualifies as a lot can happen in 4 years.0 -
NHS card will probably be the tax credit exemption card for health costs.
OP, if your friend claims DLA or had previously been in incapacity benefit (or income support in leui of incapacity) AND he had a condition that caused him to fail the tax credits disadvantage at work test (the criteria of which are in the book that comes with the claim pack) then he is entitled to the disability element of tax credits. No one from the tax credit office can look at your friends medical records to assess this.0
