You will need to apply for income based JSA. Do you have a partner who works, and/ or capital over £6,000 or £16,000?
    Gone ... or have I?

You can apply for both parts of JSA or JSA(C) only. It isnt possible to not claim JSA(C).

In any case, after JSA(C) runs out they automatically review the claim whether JSA(IB) was claimed at the start or not, they should automatically send you a JSA3 review form. Complete it and return it as soon as you get it.

They probably won't 'automaticlly' review or send a JSA3-you need to get one at the Jobcentre next time you sign on

Where I used to work in JSA it was standard to issue them two weeks before review was due.
