Invoice for treatment on Healthcare plan

I have a company healthcare plan. it is very comprehensive and no excess to pay.
i had to use the facility late last year, for my daughter. i telephoned the company, they authorised the claim and they sent me a claim form, which i filled in and took to the private hospital with me.
my daughter had a CT scan, along with other tests, all coming out clear.
last week, over 9 months later, i received a bill for £715 for the cost of the CT scan. the insurance company are saying they won't pay as they did not get the claim form in time ( 6 months ). they say the hospital in question did not foward the claim form on in time. thing is, the consultations fees were paid, so they must have had it.
does anyone have any idea what i should do or offer any advice?

It is quite possible that the consultants fee's are charged seperately from things such as scans & tests so you need to be checking with the hospital to find out whether they in fact invoiced for the consultant at a completely different time than they did for the scan.The bigger the bargain, the better I feel.
I should mention that there's only one of me, don't confuse me with others of the same name.0 -
If this is a company scheme then you might get some help from whichever department in your company liaises with the Health insurer, particularly if you work for a large company and the contract is a reasonably large one for the insurer.0

ps, I'm sure cattie is right, the Consultant would have sent his own invoice, separate from the hospital department that does the scans. Still doesn't mean you should have to pay.0
