    Stop reading other peoples horror stories - if the form is completed correctly, there is no reason why you should have a problem. If you are unsure of how to complete it you can get help from CAB, DIAL or Welfare Rights. Whilst you are waiting for an appt, you can start the process by requesting medical reports from your GP, Consultant etc.
    Gone ... or have I?
  • Many thanks for your kind words and help. Can i ask do the medical reports have to be paid for?? and should they be posted to me or direct to them?

    Whoops, gave you a thanks by mistake Mollsmum, it was intended for Dgm24.:o

    There are some GP's who will charge for writing you a personal independent medical report, so if this is what you want, you'll need to check out with yours if they levy a charge.But a standard report in support of your dla application should be free. Hospital consultants reports should be free for dla support criteria.

    Refresh the GP's mind regarding what the care & mobility needs are before asking for the report, typing them out a sheet with salient bullet points if neccesary.

    The reports are best if sent by you as it's necessary to have name & national insurance number on the top of each document. If Doc's are sending direct, make sure they have this information.
    The bigger the bargain, the better I feel.

    I should mention that there's only one of me, don't confuse me with others of the same name.

    Sorry but:rotfl::rotfl::o
  • Did you keep a copy of your last application as that's what my OT recommends me to do?

    I think you can phone & ask for a copy if you haven't.

  • Thanx very much for all the replies :)

    I have requested for them to send me the old form, so now hoping that arrives.

    Just to fill the horrible thing in! :(

    I've just had my renewal and went to CAB for assistance in completing it. I was advised to keep a copy of my forms (scanned them into my portable HDD) when I originally claimed and it was very handy to have completing the renewal one. When the renewal came back I was awarded the same as last time. However, the CAB are now going to help me with my appeal to try and have the mobility component increased (the DM never even contacted my GP this time around and she had already supported my application for a Blue Badge).

    The CAB suggested I get my GP to write a report for me and I'm thinking about asking the other Doc I see as well for some more input just to get a bit more ammo. I just feel better having the support of a claims specialist when it comes to dealing with the appeals system as I just get myself all wound up about it all - get all hot & bothered just thinking about it!
