Medical evidence for DLA claim?

Hi, i have been given some good advice on here with a previous post, and now know i will need as much medical evidence as possible to support my new claim, anyway, i am just wondering how i go about obtaining a report of my medical report which i had with an atos doctor last year, would i have to phone the esa office or atos office, its just that form has everything i was not capable of doing etc, and was why i was placed in the support group. any help appreciated.

It'll be Medical Services (Atos). I'd imagine they want it in writing but you can ring them to ask. They'll give you their address if you need to write but it should be on the letter they sent to you with details of your assessment appointment

Sorry, I don't think that an ESA report from last year would be useful to support a new DLA claim now.

File a Data Protection Act request.

Tip: Anyone who has a copy can be targeted, not just the originator.

I've found multiple requests quite useful, particularly when what is supposedly the same data turns out to be different, depending on where it comes from (it had been 'tweaked').
