Have you actually spoke to them at all? I had a phone call from social services last week who asked me what help I thought I needed and had a questionnaire done. social services should assess you for your care needs regardless of DLA award, as you may need more care than dla say you need if you get me.
you could allways go to tribunal with DLA if you think their decision is wrong you dont have to have a diagnosis to claim DLA! its the level of car and mobility needs that you claim for. as you have a whole archive of x-rays mri's and ct's if i follow you you can produce this as evidence at your tribunal.0 -

OP. We would all like to help you, but we NEED to know what you want us to answer.
If you have that level of care needs, then firstly your middle rate DLA award is wrong!
You need to stress to your GP how difficult you are struggling with everything and ask if he/she can get involved to get the ball rolling for any extra help you may need.
You need to contact your local council about your living arrangements as a top floor flat is not suitable to you by the sound of it.
You desperately need consultant support letters to back up your illnesses or care/mobility needs. OT and SW assessments can also be included in this.
I've never heard that if your on IS and DLA you need to use DLA to cover costs that have been assessed as needing?? TBH, even if this does have to be done, surely it would make more sense to look for someone private and pay them less money.
Are there any charities that would help on your case to make sure youre having all the support you require.
I pay for 7.5-10hrs a week and am pretty much bedbound and in severe pain 24/7. I have help getting out of bed, washing, toilet needs, dressing me, dirty bedding in the wash, food prep, cuppa and sarnie made for me lunch. The young lady I have comes in at 7.30am and is here just over an hour and my day is then set up with lunch done (sarnie etc) and basic meal prep like peeling spuds and carrots. All of this I pay from my DLA as thats what mine is used for! Daily I have a nurse come into to monitor me, morphine injections, check bed sores, blood tests etc etc. I have to have special cups, bed trays, cutlery, beakers, plates etc.....I am 35 and feel more like 90! My helper is lovely and leaves me with a hot drink and a sarnie in the fridge for lunch.

OP - why are you living at the top of a block of flats? I can fully understand moving can be hard, but it is clearly making your life more difficult than it ned be. You've spent £50k on private healthcare and what not, would that not have been better spent moving to a ground floor flat?
Secondly, if you were my sibling/child and were suffering as badly as you seem to say, no 'family and committments' would stop me doing simply weekly tasks for you (like shopping more economically perhaps). The welfare state hasn't always been here - how do you think people managed before then?
I'm here as I'm single and have more than 1 bedroom so I'm housed correctly & at the END of all the HA & local authority medical points lists, I cant afford to go private landlord as it eats into my E&SA as rent is expensive here unless I move to a council poverty trap and I fight depression enough without that adding to that.
No I borrowed about £40,000 on debt, living, taxi costs, rent/council tax when FIT FOR WORK by ATOS(SER), only £10,000 went private health.
I did have a 2 hour NHS MRI tonight....9 years later, only after I proved spinal arthritis in the lower back via private CT/MRI.
My siblings are both single parents working full & part time jobs with 1 on nightshift fulltime (&more), I really don't care about before I was born in the 70's, the welfare state was designed to protect people in my situation - on every count its failed me miserably.SO... now England its the Scots turn to say dont leave the UK, stay in Europe with us in the UK, dont let the tories fool you like they did us with empty lies... You will be leaving the UK aswell as Europe

Of course that is what DLA is for! All I was saying is that if the council have assessed you as needing X amount of hours and you are on Income Support, then they will either cover all or some of the costs usually. There is also Direct Payments available through most councils.
I did also suggest finding a private helper and funding yourself.

I too am unsure exactly as to what is being asked in the OP. I also have brain fog due to FM so it makes reading difficult as I tend to skim through things but I am sure I haven't seen any reference to going to Citizens Advice Bureau regarding advice as to what your actual entitlement is to benefits etc. I am sure if you were able to have someone who can act as an advocate and advise you as to what you are actually entitled to (maybe they could advise you how to get into more suitable accommodation as well). I also believe there is an organisation called DIAL who help with/give advice on welfare benefits, community care, equipment, independent living as well as mobility and transport (don't know where you live but you'll get info here www . dialuk .info/findadial/index. asp as to your local office).
I have used my local CAB for assistance with my claims and they are currently helping me to appeal my latest DLA award as my health and mobility has deteriorated a lot since my last award but the new award level has remained the same. My husband gets assistance with his claims etc from SSAFA/RBL as he is ex HM Forces (I could as well as his spouse or because I'm a former Reservist).
There is help to be had out there but you might need assistance from someone who knows all the ins and outs of "The System" to get it.0
