It's from a 2001 Ford KA, it's a:

Part Number 1S5F-18C815-DA

You are going to have to remove your radio from the dashboard in order to find the correct Serial Number beginning with M****** or V****** found on a label on the casing

Hi I need to take my radio out of my Puma but I don't have the code i need when I put it back in. i've done the 1+6 button thing and the following came up.
