Hi there & welcome
I take it the report does not confirm the insurance side of it then?
I would personally try to get the info direct from the company if you can, by either writing a simple letter, enclose a £1 cheque or postal order, and request they send you a copy of your agreement by 12 days.
The other way is you could ring the business, as long as you have the account number - they should check this out for you.
Or another way is to request a Subject access request (SAR), you enclose £10 cheque or postal order, and they have 40 calendar days to comply and send all they hold on your accounts.
There is a letter template for this on link below.

However, if you can try to find out by ringing them first, and you have been mis sold the policy, you could go ahead and make a reclaim for your reasons by completing the FOS questionnaire, they have 8 weeks to respond with a decision, sometimes they request for more time.
Keep a copy for yourself as well, and you send the questionnaire to the ones who set up and sold you the policy.
This way you may even receive a copy of the agreement free of charge enclosed with the first letter from them.
The FOS questionnaire is used instead of a template reclaim letter.
And you will find this on below link - further down the page as stage one of reclaiming.

Hope this helps, good luck.;)The one and only "Dizzy Di"
