Advice on re-housing to Council Property

I wondered if anyone knows anything about this.
I am disabled and my disability is one which means I lose more mobility etc year by year.
I currently am renting a house privately.
I have recently had an OT assessment for home aids and the OT has said that I need to move to supported accomodation as it is no longer safe for me to stay in the house where I live at the moment due to falls etc and the inability to make changes to the house to make it safer for me, i.e. a wet room to replace the current bathroom.
She (along with GP, Consultant and Social Services) has written to the Council to state that I am no longer safe in my home and need supported housing.
It's been suggested to me that once the Council are in receipt of these facts does this then become a legal duty of care from them. Is this correct?
Any info/advice would be greatly appreciated.

Speak to someone like DIAL (disability advice) or Shelter who are experts in this area and will know about the council's obligation to the disabled and whether its a discretionary or statutory obligation to give you sheltered housing.0
