a little bit of help.

Im was going to try and fill in this form by myself, afterall im not lying about anything do ill only be telling the truth.

The form is for my daughter and im a little comfused about a couple of things.

Who would be the best person to contact for help , is it the benefits office?

    The section for a 'statement that knows you well' - maybe a nurse, doctor or someone who know what problems your child has on a day to day basis could fill this in for you.

    Try to write as much info as possible, and dont worry if you feel you are repeating yourself. You need to clearly illustrate that your child has more care and/or mobility needs than a child of the same age without their condition.

    Speak to all the people who are involved with your childs care - school (if applicable), hospitals, doctors etc etc as they may well be contacted.

    Make sure you enclose any addtional info like hospital letters, prescription forms, doctors notes etc to back up your comments on the form.

    If you feel happy to fill out the forms then go ahead - if not contact welfare rights via your local council or the CAB.

    well if i post on here the bits im stuck on maybe somebody here can help me:

    part 5 says more about te child and asks you to 'tick' boxes that apply to them.

    She has physical disabilites os i can tick that but it says leaning difficulites but we are not really sure if she does yet. She has been really set back so she cannot crawl. stand or walk ect. She cannot do things a child her age should (she is 19 months) a symptom of her disease is slower to learn so im not sur eif to tick this box. She cant talk yet either and still 'plays' with abby toys.

    the next bit is 'walking outdoors ans does the child need someone with them' as she is not old enought for mobility companent so i still need ot fill these sectiosn in?

    Also the section where it says how often per day do they need care. I cannot leave her at all as she cannot support herself so what do i put here??

    Any help would be greatfull recieved.

    DWP don't see walking as a problem until the age of 3 years.

    Because she's 19 months, they will argue that any child of that age would need constant supervision. An average 19 month old might just need less adult input.

    Thats what im askin do i just leave that section about walking outdoors out.?

    im not sure what you meant by the last section, she needs a lot more care than an adverage 19 month old. She cant do anything a child or that age would.


    how do i put that down in minutes when she needs me all day.?

    You might be better off timing it for a week or two and put down the average.

    You leave any sections that aren't relevant to her.

    thanks sh1305 - the walking is relevant - because she cant, but i also understand that is is not eligable for mobility until 3, so i should just leave this section out. I just dont want them to think she CAN walk, when she cant.

    Might try the timing thing, it sjust difficult with her, becasue the only time she can be left alone is when she is asleep, anyother time i am either helping her do something or she is in one of her pieces of equipement which she cannot be left in by herself.

    Personally i wouldnt leave out the section on mobility - although she may not qualify for the mobility component, if you fill it in it will give the decision make a fuller picture of her condition and needs, and that she has these needs as well as the care needs.

    As far as timing it - write down what you wrote on the above post - that it is difficult to put minutes because etc ....

    Put as much info as possible and describe in detail a typical day so they can see how that compares to another typical child without the condition of the same age.

    For the timing, you can put down "today, it took 10 minutes, yesterday it took 20", etc. By doing this, you're also giving them a good picture of what better, average and bad days are like. There are boxes to put down additional information - don't rely on the tick boxes or the decision maker to know what you mean. Whilst it's obvious to you, it may not be obvious to them.
