A CAF an be a great way of identifying what the needs of individual children are and getting appropriate people to work together. If you go to your local authority website and type in CAF you should get some answers as to how it works in your area. However any proffessional who works with your children should be able to start the caf process. I would however suggest health visitor or school are often best at actually doing it.0

basically its a failry long form that is filled in (usually started by the school or a HV) its to identify any and all the needs that a child has. it is then sent out to multi agencies (assuming u agree it can) so that they can all have input into those needs
unfortunately it does not guarantee those needs will be acted on or met but its a start in the right direction

Do your boys have a Social Worker? If they do then a CAF wouldn't proceed while the Social Worker is still involved. Otherwise I'd suggest your Health Visitor. After the professionals met they awarded my son funding for 20 hours of pre-school every week from his CAF, it's done wonders for him.
