    Thanks andyandflo - Im sorry to hear you lost your case - I lost my first one and even on the second one my Doctor wrote a letter to the effect that Mrs ****, my patient tells me she can not do "x", "y" and "z" because of her "illness" etc - they kept rejecting it - in the end the Dr was really getting quite cross - do you know why .... because he had not put "it is my opinion she canot do "x" "y" "z" because of her "illness" - see the difference - both statements were completely honest its just he had said I said and not he knows I cant etc. I thought the forum was meant for help and support not blame so the comment made to you was completely uncalled for and of no help whatsoever - probably made you feel even worse - so I am sorry that you have had a rough time - but thanks for your words of support they meant a lot :T

    Thanks also to the two members who said I should ask for a copy of my old form from DWP duh womanly moment there - had asked welfare rights but did not think DWP would let me have it - will sort that tomorrow

    Not only did I originally post for help overall but was hoping someone may come up who has filled in a form or had help when suffering with a MH issue

    I agree, getting a copy of your original claim form is the best place to start.
    Do you have a D I A L (Disability Information and Advice Line) in your area?
    Theyre very good at wording the forms and giving all the relevant information.

    Before you do the questions, get some paper and go through every relevant question. Break the tick boxes into sections. For example, on the question regarding supervision when out, there are boxes that mention panic attacks and wandering off. Why do you have panic attacks? why do you wander off? Also write down why someone being with you will help. IE, "I need someone with me because I wander off. If I wander off, I have panic attacks and need someone to reassure me and calm me down." You may also want to give examples of this.

    Write as much detail as possible and never assume that the DM knows about your condition. They just go by what you write on the forms. If you can, get someone who knows you to write a statement detailing the care they give you.
