Jobcentre lost application form

Hi there,
My wife and I completed a Sure Start Maternity Grant form some weeks ago. As instructed we handed the form into our local Jobcentre Plus office within the 3 month deadline (I handed it to the reception staff - with hindsight I should have asked for a receipt for it).
Well some weeks passed without any response so we contacted the office which processes the forms, they say they have not received it. We contacted the Jobcentre Plus office we handed it in to and they say then cannot find any mention of it in their 'post log'.
The problem is that there is a 3 month deadline for handing in the application form, which has now past.
We have another form, which we will fill in, we are going to be returning to the original Health Visitor who signed the first form, and will ask her to put a note on it saying she did complete the form within the deadline first time around.
The person my wife spoke to in the processing office was dubious that we would receive the money, since the deadline has past
. The staff in the Jobcentre Plus do not want to get involved since they say they have no record of receiving it in time.
Does anyone have any advice on anything further we can do?
