What do I do next to get my money back from Orange?

In March I bought a new PAYG phone from the Orange website. I had problems with it and despite phoning lots and lots, the credit that came with the SIM card was never applied. I sent the phone back but they only refunded me for the phone and not the £10 credit that came with the SIM card and which I never received. I made lots more phone calls and was promised the outstanding £10 would be refunded but again nothing happened.
I sent a formal letter of complaint two weeks ago, I followed the format suggested on the Ofcom website. I haven't heard back from Orange.
What do I do next?

Had the same problem myself - no joy from customer services whatsoever, so I emailed Tom Alexander Chief Exec of Orange on Tom.Alexander@orange.co.uk problem solved within 24hrs
