Direct Debit Charges?

Hi I'm hoping someone can help me.
My grandad is currently in hospital (he is blind, deaf, has diabetes and many other illnesses) He is moving into a nursing home within the next week.
Up until now his friend who is also his carer has looked after his bank account and all his money. With him going into a Nursing home my mum is going for Power of Atourney and is in the process of sorting out his bank account so that everything is in order when he gets moved. She took a mini statement from a cash machine on friday and it had a Direct Debit out of it which she knew about but also a £5 charge. When she asked my Garandad's carer what the £5 charge was she said it was a bank charge for the Direct Debit.
Can the banks charge you for a direct debit coming out of your account? If not how would my mum go about claiming the money back?
Any help would be very much appreciated.

the only charge related to a DD would be if he was over his credit limit - which wouldnt be £5.
suggest look at the full bank statement

He is not overdrawn there is money in his account to cover his bills as he has been in hospital for most of this year.
She only has 2 previous statements that my grandad's carer gave her, will ask her to check the statements she has to see if there is any more detail on them.DFW Nerd no. 177
