Stump Grinder Advice Please

I'm going to hire a stump grinder to remove tree stumps but have a couple of questions:
Do tree stumps need to be dead before using a stump grinder
How big a stump (height) will a stump grinder remove

You want to cut the stump down as low as possible I would think 2 inches or less sticking up would be a good height.

I recently had a stump ground, using his smaller (still big!) machine he ground it well below ground level so in my case it can be paved over.0
