Grant for Alteration

Does anyone know where I can get information for a grant for alterations on homes.
My dad is in hospital and is due to come home in the near future. They don't have a down stairs toilet at present or down stairs bedroom and my dad really could do with these. The hospital have mentioned about a grant for alterations but then never mentioned anything else.
If anyone know where I can find out about this.

Contact local council Social Work dept and ask for an occupational [STRIKE]health[/STRIKE] therapy referral.A kind word lasts a minute, a skelped erse is sair for a day.0

Be warned that there is normally a waiting list to get an appointment for sn Occupational Therapist to come and do an assessment.
Local authority also need to have 3 estimates from their list of "approved contractors" before they make the decision and the work can start.
I needed a stair-lift and a level access shower after I cam out of hospital in the July but the work was not started until the following January and completed by April.0
